My Health Passport is a must-have document that describes important aspects about a person's special health care needs, all in one place. This version was adapted by Dr. Elizabeth Perkins at the Florida Center for Inclusive Communities.

My Health Passport was designed to be shared with many types of healthcare providers, in clinic and hospital settings. It is useful for providing pertinent information to those who are not very familiar in providing care to individuals with developmental disabilities and communication difficulties.

The Health Passport is four pages long and includes sections describing communication methods, medical history, current medications, allergies, pain, managing distress, mobility needs, self-care, eating, drinking, favorites food and drinks, sensitivities, leisure activities and more. It is available in both English and Spanish versions.

The PDF form is customizable so you can download it to your computer, fill it in, print it and save it to be updated as necessary.
